The hospital was founded in the early 20th century by Montreal pharmacist Henry J. Dart, who named it in honor of his daughter Grace, who died of tuberculosis in 1898. The hospital first opened its doors in 1907 on Sainte-Antoine Street. It later relocated to Sherbrooke Street, where the new Grace Dart Home Hospital, designed by architect David R. Brown in 1930, was inaugurated in 1932. For over 60 years, the 101-bed hospital operated as a sanatorium.

In 2016, due to its deteriorating condition, the building—then called the Grace Dart Pavilion—was closed, and residents were transferred to two other healthcare facilities: the Ste. Anne’s Hospital for Veterans in the West Island and the Grace Dart Extended Care Centre on Sainte-Catherine East, affiliated with the CIUSSS de l’Ouest-de-l’Île-de-Montréal.

Since its closure, the complex—owned by the Extended Care Centre—has remained vacant, and its future remains uncertain, although it has been identified by the City of Montreal as a building of exceptional heritage value.

In 2021, a redevelopment project led by TGTA was announced, prompting some criticism from local residents. The project planned for 350 housing units, including 20% social housing (managed by a cooperative), to be distributed between the existing building and a new construction at the rear of the former hospital. However, funding challenges for the social housing component led to the project being put on hold.

In 2023, developers Maître Carré and TGTA partnered with Fiera Real Estate and Fondaction to move forward with the redevelopment of the former Grace Dart Hospital, with the goal of transforming it into a complex of 299 rental units.

The conversion was completed in 2025.
